- Police Loss Report for the lost certificate.
- The page of the Government Gazette containing the announcement of the lost certificate.
- A letter from the Fire and Rescue Force or local government authority regarding the burning/loss of the certificate due to fire, floods, termites, rats, or any other damage.
- Remnants of the damaged certificate (if available).
- One copy of an Identification Card (National ID, Voter’s ID, Driving License, or Passport).
- Photograph (Passport size).
